Vendors that are interested in becoming a qualified Vendor of Record for projects under $10,000 must full out and return the form below to the Purchasing Department. Responses will be evaluated and those vendors that meet the qualifications will be placed on the appropriate list to be called upon directly or to receive quotations when required.
